iwlwifi: fix set_tx_power vs scan

According to comment in iwl_bg_scan_completed, setting tx power should
be deferred during pending scan, but we are not doing this.

This patch change code to really defer setting tx power after scan
complete. Additionally refactor iwl_set_tx_power code and call
lib->send_tx_power() directly from iwlagn_commit_rxon.

ath9k_hw: fix potential spurious tx error bit interpretation

According to documentation, AR_ExcessiveRetries, AR_Filtered and
AR_FIFOUnderrun are only valid if AR_FrmXmitOK is clear.

Not checking this might result in suboptimal FIFO settings, unnecessary
retransmissions, or other connectivity issues.

ath9k: add a debug warning when we cannot stop RX

We have seen several DMA races when we race against
stopping and starting the PCU. I suspect that when
we cannot stop the PCU we may hit some of these same
races so warn against them for now but only when
debugging (CONFIG_ATH_DEBUG) is enabled.

If you run into this warning and are a developer,
please fix the cause of the warning. The potential
here, although I cannot prove yet, is that the DMA
engine can be confused and start writing to a buffer
that was already DMA'd before and at least the kernel
assumes is not being accessed by hardware anymore.

cfg80211: fix disabling channels based on hints

After a module loads you will have loaded the world roaming regulatory
domain or a custom regulatory domain. Further regulatory hints are
welcomed and should be respected unless the regulatory hint is coming
from a country IE as the IEEE spec allows for a country IE to be a subset
of what is allowed by the local regulatory agencies.

So disable all channels that do not fit a regulatory domain sent
from a unless the hint is from a country IE and the country IE had
no information about the band we are currently processing.

This fixes a few regulatory issues, for example for drivers that depend
on CRDA and had no 5 GHz freqencies allowed were not properly disabling
5 GHz at all, furthermore it also allows users to restrict devices
further as was intended.

If you recieve a country IE upon association we will also disable the
channels that are not allowed if the country IE had at least one
channel on the respective band we are procesing.

This was the original intention behind this design but it was
completely overlooked...

cfg80211: fix allowing country IEs for WIPHY_FLAG_STRICT_REGULATORY

We should be enabling country IE hints for WIPHY_FLAG_STRICT_REGULATORY
even if we haven't yet recieved regulatory domain hint for the driver
if it needed one. Without this Country IEs are not passed on to drivers
that have set WIPHY_FLAG_STRICT_REGULATORY, today this is just all
Atheros chipset drivers: ath5k, ath9k, ar9170, carl9170.

This was part of the original design, however it was completely
overlooked...

ssb: Clear RETRY_TIMEOUT in PCI Configuration for normal devices

MMIO log traces obtained using the Broadcom wl hybrid driver show that
the RETRY_TIMEOUT register (0x41) in PCI configuration space is cleared
if non-zero. Similar code found in other drivers such as ipw2100 show
this operation is needed to keep PCI Tx retries from interfering with
C3 CPU state. There are no known cases where omission of this code has
caused a problem, but this patch is offered just in case such a situation
occurs.

ath9k_hw: optimize all descriptor access functions

Because all of the descriptor data structures are marked as __packed, GCC
assumes the worst case wrt. alignment and generates unaligned load/store
instructions on MIPS for access to all fields.
Since descriptors always have to be 4-byte-aligned, we can just mark the
data structures with __aligned(4), which allows GCC to generate much more
efficient code.
Verified through disassembly and OProfile comparisons.

ath9k: Convert to new PCI PM framework

The ath9k driver uses the legacy PCI power management (suspend
and resume) callbacks that apparently cause intermittent problems
to happen (the adapter sometimes doesn't resume correctly on my
Acer Ferrari One).  Make it use the new PCI PM and let the PCI core
code handle the PCI-specific details of power transitions.

ath9k: optimize/fix ANI RSSI processing

ANI needs the RSSI average only in station mode, and only for tracking
the signal strength of beacons of the AP that it is connected to.
Adjust the code to track on the beacon RSSI, and store the average of that
in the ath_wiphy struct.
With these changes, we can get rid of this extra station lookup in the
rx path, which saves precious CPU cycles.

ath9k_hw: optimize tx status descriptor processing

Disassembly shows, that at least on MIPS, the compiler generates a lot of
memory accesses to the same location in the descriptor field parsing.
Since it is operating on uncached memory, this can be quite expensive in
this hot path.
Change the code a bit to help the compiler optimize it properly, and get
rid of some unused fields in the ath_tx_status struct.

ath9k_hw: small optimization in ar9002_hw_get_isr

ah->config.rx_intr_mitigation does not need to be checked before checking
the rx interrupt mask for AR_ISR_RXMINTR or AR_ISR_RXINTM, as those
interrupts will be masked out if rx interrupt mitigation is disabled.

Avoid reading AR_ISR_S5_S twice by reordering the code to be more concise.

ath9k_hw: optimize interrupt mask changes

OProfile showed that ath9k was spending way too much time in
ath9k_hw_set_interrupts. Since most of the interrupt mask changes only
need to globally enable/disable interrupts, it makes sense to split
this part into separate functions, replacing all calls to
ath9k_hw_set_interrupts(ah, 0) with ath9k_hw_disable_interrupts(ah).

ath9k_hw_set_interrupts(ah, ah->imask) only gets changed to
ath9k_hw_enable_interrupts(ah), whenever ah->imask was not changed
since the point where interrupts were disabled.

i2c-i801: Handle multiple instances instead of keeping global state

It's poor form to keep driver state in global variables rather than
per-instance. It never really mattered in practice when there was only
one controller on the chipset, but the latest chipsets do have more
than one controller, so now we care.

locks: let the caller free file_lock on ->setlease failure

The caller allocated it, the caller should free it.

The only issue so far is that we could change the flp pointer even on an
error return if the fl_change callback failed.  But we can simply move
the flp assignment after the fl_change invocation, as the callers don't
care about the flp return value if the setlease call failed.

done as a 64-bit quad.  On little-endian (like x86) this doesn matter,
but on a big-endian machine the 32-bit reads will see the (zero) high
bits.

Only mips, s390, and score set CONFIG_32BIT for 32-bit builds, so fix it for
all other 32-bit arches by inverting the logic and testing for CONFIG_64BIT,
which should be defined on all 64-bit arches.

nfsd4: initialize delegation pointer to lease

The NFSv4 server was initializing the dp->dl_flock pointer by the
somewhat ridiculous method of a locks_copy_lock callback.

Now that setlease uses the passed-in lock instead of doing a copy,
dl_flock no longer gets set, resulting in the lock leaking on delegation
release, and later possible hangs (among other problems).

So, initialize dl_flock and get rid of the callback.

locks: prevent ENOMEM on lease unlock

Removing a lock shouldn't require any allocations; a failure due to
ENOMEM leaves the caller with a choice between retrying or giving up and
leaking an unused lease.

Next we should split the other lease calls into add and delete cases.
I wanted to start with just the bugfix.

RDS: Let rds_message_alloc_sgs() return NULL

Even with the previous fix, we still are reading the iovecs once
to determine SGs needed, and then again later on. Preallocating
space for sg lists as part of rds_message seemed like a good idea
but it might be better to not do this. While working to redo that
code, this patch attempts to protect against userspace rewriting
the rds_iovec array between the first and second accesses.

The consequences of this would be either a too-small or too-large
sg list array. Too large is not an issue. This patch changes all
callers of message_alloc_sgs to handle running out of preallocated
sgs, and fail gracefully.

RDS: Copy rds_iovecs into kernel memory instead of rereading from userspace

Change rds_rdma_pages to take a passed-in rds_iovec array instead
of doing copy_from_user itself.

Change rds_cmsg_rdma_args to copy rds_iovec array once only. This
eliminates the possibility of userspace changing it after our
sanity checks.

Implement stack-based storage for small numbers of iovecs, based
on net/socket.c, to save an alloc in the extremely common case.

Although this patch reduces iovec copies in cmsg_rdma_args to 1,
we still do another one in rds_rdma_extra_size. Getting rid of
that one will be trickier, so it'll be a separate patch.

net: fix rds_iovec page count overflow

As reported by Thomas Pollet, the rdma page counting can overflow.  We
get the rdma sizes in 64-bit unsigned entities, but then limit it to
UINT_MAX bytes and shift them down to pages (so with a possible "+1" for
an unaligned address).

So each individual page count fits comfortably in an 'unsigned int' (not
even close to overflowing into signed), but as they are added up, they
might end up resulting in a signed return value. Which would be wrong.

Catch the case of tot_pages turning negative, and return the appropriate
error code.

This patch fixes the following section mismatch warning:

This makes sense because the variable is of the type "struct pci_driver".

Fix up trivial conflicts in fs/btrfs/super.c and fs/fs-writeback.c, and

fs-writeback.c: unify some common code

The btrfs merge looks like hell, because it changes fs-writeback.c, and
the crazy code has this repeated "estimate number of dirty pages"
counting that involves three different helper functions.  And it's done
in two different places.

Just unify that whole calculation as a "get_nr_dirty_pages()" helper
function, and the merge result will look half-way decent.

fs/compat.c: fix build on MIPS/s390

The definition of PAGE_CACHE_MASK in <linux/pagemap.h> is needed to use
MAX_RW_COUNT, and on x86-64 that gets done indirectly through the
architecture header includes.  But on MIPS and s390 that doesn't happen,
and we need to make sure that fs/compat.c includes pagemap.h explicitly.

Introduced in commit 435f49a518c7 ("readv/writev: do the same
MAX_RW_COUNT truncation that read/write does").

Normal syscall audit doesn't catch 5th argument of syscall.  It also
doesn't catch the contents of userland structures pointed to be
syscall argument, so for both old and new mmap(2) ABI it doesn't
record the descriptor we are mapping.  For old one it also misses
flags.

audit: Call tty_audit_push_task() outside preempt disabled

While auditing all tasklist_lock read_lock sites I stumbled over the
following call chain:

audit_prepare_user_tty()
  read_lock(&tasklist_lock);
  tty_audit_push_task();
     mutex_lock(&buf->mutex);

     --> buf->mutex is locked with preemption disabled.

Solve this by acquiring a reference to the task struct under
rcu_read_lock and call tty_audit_push_task outside of the preempt
disabled region.

Move all code which needs to be protected by sighand lock into
tty_audit_push_task() and use lock/unlock_sighand as we do not hold
tasklist_lock.

mtd: fix build error in m25p80.c

While building an x86 distro kernel, I hit the following:

Kernel: arch/x86/boot/bzImage is ready  (#7)
ERROR: "of_mtd_parse_partitions" [drivers/mtd/devices/m25p80.ko]
undefined!

of_mtd_parse_partitions is defined with MTD_OF_PARTS, and that's only
built on PPC and microblaze.  The code in question should be wrapped w/
a stricter #ifdef.

mtd: Remove redundant mutex from mtd_blkdevs.c

In commit 2a48fc0ab24241755dc93bfd4f01d68efab47f5a ('block: autoconvert
trivial BKL users to private mutex'), Arnd replaced the BKL usage with a
mutex. However, Maxim has already provided a better fix in commit
480792b7bf188c29b8d4b10fee65c3a06ec5dbf7 ('mtd: blktrans: kill BKL'),
which was simply to remove the BKL without replacing it — since he'd
already made it do all necessary locking for itself.

During unlink we remove any references to the inode from
the tree log.  It can return -ENOENT and other errors,
and this changes the unlink code to deal with it.

MTD: Fix wrong check register_blkdev return value

register_blkdev return 1..255 when major = 0.

Above code will return fail when register_blkdev return allocated major number.

semaphore: Remove mutex emulation

Semaphores used as mutexes have been deprecated for years. Now that
all users are either converted to real semaphores or to mutexes remove
the cruft.
