Subject: [PATCH] [SCSI] qla2xxx: Cleanup any outstanding SRB resources during
 shutdown.

Refactor SRB-failure completion codes in the process.  Also,
signal the DPC routine to complete sooner as backend processing
at shutdown-time is superflous.
