musb_hdrc cleanup -- comments, dates

Clarify dates in comments ... they are from 2006.  Drop reference
to the Mentor "2.2a" code base; it's not remotely relevant any more.

Also fix up a few comments, and remove a few "sparse" warnings,
mostly on DaVinci.
